Lena had been posing as Esther to get close to the Klammer family with the intention of killing them and taking their daughter, Max (played by Aryana Engineer), for herself. It is revealed that Lena has been doing this for years, assuming different identities to get close to families and steal their children.

The climax of the movie sees Lena attempting to kill the Klammer family and take Max for herself. She is ultimately unsuccessful, but not before she kills several people and causes immense damage. The movie ends with the Klammer family broken and traumatized, while Lena is back in a mental institution, plotting her next move.

Overall, "Orphan: First Kill" is a chilling and suspenseful movie that keeps viewers on the edge of their seats from start to finish. The performances by Fuhrman and Stiles are outstanding, and the movie does an excellent job of building tension and suspense.
